Covid Policy *

Parents' Code of Conduct *- All parents are required to submit this form. A digital version was included in the registration process; if that was completed then a hard copy is not necessary.

Pre-Participation Physical Form *- This medical form must be completed by a medical provider and is REQUIRED before any player can participate in any Hereford Youth Football game. All medical forms are due to your team manager by August 15th.
